What You Need Before Your First Podiatric Appointment

To ensure a smooth appointment process, we kindly request that you complete our patient registration process before your appointment. You can do this through our patient portal, which we will supply you with a login. Once you have completed the process, our office will be notified electronically.

Alternatively, please review the links below if you prefer to complete the forms in writing. When you click on the links, a separate browser window will open. From there, you can click on the PRINT icon to access the new patient registration and HIPAA Privacy forms. You can then complete these forms and either bring them with you on the day of your appointment or fax them in advance to (636) 928-1242 to save time.
